Summary:
The Infusion Coordinator utilizes clinical skills in a collaborative process to identify, coordinate and facilitate exceptional healthcare outcomes for patients. Responsible for coordinating care for complex infusible drug therapies. This position works with the prescriber, pharmacy clinical staff and nursing staff to organize infusion care for the patient.
Detailed responsibilities:
• Perform individualized clinical screening assessments of patient’s disease and therapy and responsible for care coordination for the administration of prescribed infusible drug therapies. Reviews patient supplies, IV line access, flushing protocols and accesses patient understanding of the care process. Ability to assist in the coordination of medication deliveries and nursing care.
• Obtain and track insurance authorizations for infusion services for patients. Track referral status beginning in the inpatient unit to dispensing, to ensure prescribed therapy regimens are administered to patients in a timely manner.
• Provide ongoing patient monitoring and reassessment activities to continually assess for response to treatment and patient adherence. Assist with completion of ADR/FDA MedWatch form for all reported ADRs within a 24-hour timeframe of the incident. Complete product compliant forms within a 24-hour timeframe of the issue.
• Assist with the monitoring of patient lab results, equipment and doctor appointments. Communicates with patients and other healthcare team members to ensure availability of staff to meet patients’ needs and treatment orders.
• Act as main liaison between the pharmacy, referral resources, patient and family members. Communicates with patients and/or their families to provide necessary pre-procedure instruction. Provide patient education and resources pertaining to patient specific disease state and therapy regimens. Maintain patient records to ensure proper documentation is obtained and entered in pharmacy’s systems.
• Participate in quality program initiatives and improvement activities.
