Summary:

• Plans, implements, and evaluates patient care based on patient assessment to optimize outcomes and maximize available resources. Monitors, documents, and communicates patient condition as appropriate.

Home Infusion Coordinator: Home Infusion

  


Summary:
The Infusion Coordinator utilizes clinical skills in a collaborative process to identify, coordinate and facilitate exceptional healthcare outcomes for patients. Responsible for coordinating care for complex infusible drug therapies. This position works with the prescriber, pharmacy clinical staff and nursing staff to organize infusion care for the patient.

Detailed responsibilities:
• Perform individualized clinical screening assessments of patient’s disease and therapy and responsible for care coordination for the administration of prescribed infusible drug therapies. Reviews patient supplies, IV line access, flushing protocols and accesses patient understanding of the care process. Ability to assist in the coordination of medication deliveries and nursing care.
• Obtain and track insurance authorizations for infusion services for patients. Track referral status beginning in the inpatient unit to dispensing, to ensure prescribed therapy regimens are administered to patients in a timely manner.
• Provide ongoing patient monitoring and reassessment activities to continually assess for response to treatment and patient adherence. Assist with completion of ADR/FDA MedWatch form for all reported ADRs within a 24-hour timeframe of the incident. Complete product compliant forms within a 24-hour timeframe of the issue.
• Assist with the monitoring of patient lab results, equipment and doctor appointments. Communicates with patients and other healthcare team members to ensure availability of staff to meet patients’ needs and treatment orders.
• Act as main liaison between the pharmacy, referral resources, patient and family members. Communicates with patients and/or their families to provide necessary pre-procedure instruction. Provide patient education and resources pertaining to patient specific disease state and therapy regimens. Maintain patient records to ensure proper documentation is obtained and entered in pharmacy’s systems.
• Participate in quality program initiatives and improvement activities.

Client Facing Responsibilities

    • Primary relationship will be with Vice President Level client contacts running the client program with an expectation to build secondary relationships with Director level client contacts
  • Responsible for articulating how the Arise virtual customer contact services and business process outsourcing services will create high-value business results through superior virtual solutions.
  • From personal contacts and research within the targeted verticals, identify targets where Arise can create the most value and penetrate the C-Suite of the targeted corporations. Develop tactical plans that clearly define objectives, goals, schedules and assignments to implement targeted opportunities.
  • Develop sales process for partner calls and monitor results against plan. Evaluate wins and losses and make needed process or tactical changes.
  • Work consultatively with the executive and technical teams, at multiple levels of the targeted businesses, to develop and close large volume, high value deals.
  • Continuously develop newly acquired accounts into ever more important and substantial relationships, by championing those accounts within Arise and by constantly looking to find new ways that Arise can assist them.
  • Mange over $30M in annual revenue
  • Oversees the strategic management of the client portfolio anticipating when outcomes will not be met and leading team to perform deep analysis of root cause issues on client accounts
  • Fully understands the expected financial outcomes of all client contracts and the actions required to deliver optimal outcomes
  • Responsible for ensuring Call Volume & Gross Profit margin targets across the client portfolio are set and met
  • Ensures that direct reports adhere to established processes for developing detailed Client Account Plans and client scorecard performance measures and takes action to deliver against the contract requirements
  • Oversees the execution of weekly scorecard performance reviews and rolling this up to monthly and quarterly business reviews with each client

  • Provides leadership in architecting the strategy that ensures delivery throughout the entire lifecycle:
    • Service Partner Marketing:
      • Responsible for leveraging Service Partner Marketing to develop a profile of the characteristics of a successful agent for each client application
      • Responsible for providing accurate forecasts in order to ensure the pipeline of agents meets the need for each client application
    • Learning Design & Delivery:
      • Responsible for providing accurate forecasts in order to ensure that the appropriate number of agents are certified to operate on each client application.
      • Responsible for ensuring the delivery of quality curriculum and instructional materials within budget to achieve top performance
    • Planning and Scheduling:
      • Responsible for providing accurate forecasts in order to ensure each client application is appropriately staffed at each interval
      • Responsible for ensuring service level agreements (SLAs) are being met and that the forecasting process optimizes both the client’s and Arise’s results
    • Customer Success Managers:
      • Responsible for architecting the strategy that will drive results through agents in achieving the contractual metrics for each client application.

Qualifications

  • Coordinate with both Sales and Operations to seek operational efficiencies and cost savings (e.g., supplier/SKU optimization, variable service and delivery levels/fees, etc.)
  • Support the Supply Chain Supplier Transformation team in implementation of projects to generate shared supplier and distributor savings
  • Support the Sr. Manager of Commercial Sales Planning in coordination of resources and information from Regions and Markets throughout the process of a collaborative annual bottom-up target build process alongside the supplier
  • Serve as the single point-of-contact to coordinate and facilitate corporate initiatives, projects, and efforts (e.g., tail and incubation brand strategies, transition planning, enterprise-wide initiatives, etc.)
  • Develop analysis and presentations in support of major supplier meetings (e.g. top-to-tops)

Qualifications
Minimum Qualifications

  • Bachelor’s degree in related field and MBA; or an equivalent combination of education and experience
  • 10 years of relevant experience
  • Possess the ability to take initiative, proactively look for solutions, work and manage projects within tight deadlines, and communicate to project leadership any challenges/issues in a clear and timely fashion
  • Must demonstrate a structured, analytical thought process
  • Outstanding oral and written communication skills
  • Must be able to sustain performance under conditions of stress—such as tight deadlines and detailed questioning
  • Demonstrate strong critical thinking capabilities and problem-solving skills
  • Demonstrated knowledge of MS Office Suite and/or software applications related to job functions

Preferred Qualifications

  • Experience in reengineering, revenue enhancement, cost containment, and restructuring projects
  • Comfortable interpreting and manipulating large amounts of data
  • Supplier relationships and/or knowledge of and experience in selling the portfolio of supplier spirits
  • Management consulting experience; knowledge and experience in implementation of Problem Solving Methodology and Process Management principles
